    The Chronic Care Management Programme is a one-to-one, personalised virtual lifestyle care programme designed for customers with a clinical diagnosis of a chronic condition*. Coaching is provided by a dedicated Physician Health Advisor**, Registered Nurse, and Dietician to support effective condition management and minimise the risk of future complications.

    This service is not meant to provide you with medical treatment nor to create a doctor-patient relationship, but to guide you in changing your lifestyle to improve your health conditions.


    (*) The covered conditions are hypertension (high blood pressure), prediabetes, diabetes, and hyperlipidemia (high cholesterol).
    (**) Physician Health Advisors are clinically active senior physicians/ specialists with 10+ years of experience. They are appropriately accredited for the clinical situation and geographical coverage.

    Your Physician Health Advisor* will reach out by phone or email to coach and support lifestyle and behavioural changes that can improve your health.

    For the avoidance of doubt, the service is not meant to provide you with healthcare and/or medical treatment nor to create a doctor-patient relationship, but to guide you in changing your lifestyle habits to improve your health conditions.


    (*) Physician Health Advisors are clinically active senior physicians/ specialists with 10+ years of experience. They are appropriately accredited for the clinical situation and geographical coverage.

    This programme is designed to support you in making lasting lifestyle habits over 12 months, including weekly follow-ups for the first 2 months, and monthly follow-ups for another 10 months.

    Physician Health Advisor (GP)

    First call must be conducted via Teams video call, subsequent sessions will be conducted virtually, either via a Teams video call or phone call.

    To join the programme, you will need:

    • An active PRUApex International Health insurance plan;
    • A prior diagnosis of one of these conditions: hypertension (high blood pressure), prediabetes, diabetes, or hyperlipidemia (high cholesterol); and
    • Medical results and reports of the relevant conditions that are issued no more than 3 months ago:

    • Fasting glucose ≥ 7.0 mmol/L
    • HbA₁c ≥ 6.5%

    • Blood pressure ≥ 140/90 mmHg
    Hyperlipidaemia (High Cholesterol)
    • Total cholesterol > 5.2 mmol/L
    • LDL cholesterol ≥ 3.4 mmol/L
    • HDL cholesterol < 1.0 mmol/L
    • Triglycerides ≥ 1.7 mmol/L

    No, there is no additional fee to join the Chronic Care Management Programme.

    If you have been diagnosed with hypertension, prediabetes, diabetes, or hyperlipidemia, you are eligible to enroll via one of the following options:

    We will ask for a few basic details — your full name, date of birth, and policy number — as shown in your PRUApex International Health plan. Your policy number can be found in your e-card in your secured MyHealthWallet web/member App.

    Once your enrolment is complete and you are confirmed as eligible, your Physician Health Advisor will contact you by phone or email to guide you through every step of your health journey.

    No, Teladoc’s Physician Health Advisors do not prescribe medication.

    If you require medication for your existing chronic condition, please speak with your treating doctor or specialist.

    You may approach Prudential for a premium review if your hypertension or hyperlipidaemia condition has significantly improved. Please note that the result of the review is based on the latest information provided on your condition and the decision is at the discretion of Prudential. Prudential reserves the right to ask for any further medical evidence or examination as proof of the condition.

    The tests, treatments, procedures, devices or medication recommended by the Physician Health Advisor may or may not be covered by Policy with Prudential Assurance Company Singapore. Customers are advised to confirm the policy coverage with Prudential before undergoing any services.
